Workbench XSL templates should specify xsl:output type of HTML

Description

The workbench application serves pages as SPARQL Result XML documents, and uses XSL stylesheets for client-side transformation of those documents into HTML pages that can be rendered. At present, the xsl:output attribute either doesn't get set, or gets set to XML. This prevents a proper global document object from being created, and prevents many common JavaScript operations from working. Setting xsl:output to HTML fixes this.
